What is the CHM Hub®?

- The CHM Hub® is a patented technology platform that identifies health-related issues affecting communities and assists organizations in effectively managing population health challenges. CHM Hub® integrates a diverse suite of information with geospatial and proprietary analytics to generate insights that are leveraged to improve health outcomes at the community level. The CHM Hub® enables users to match organizational resources to localized needs through a granular understanding of the community experience.
- CHM Hub® was upgraded to version 3 in April 2020 to enhance the features and functionality of the platform. To access the latest features and functionality, please ensure you are logging in at https://insights.chmhub.com/users/login.
Who can access to the CHM Hub®?

- The CHM Hub® is made available to BCBS Plans and select accounts. For more information about who can access the CHM Hub®, please contact us at chmhub@bcbsa.com.
What additional services does CHM Hub® offer?

- The CHM Hub® platform users can access additional services:
- Community Insights: Identification of population-specific barriers to health.
- Account Support: Identification of social determinants of health specific to accounts and the geographies in which employees work and live.
- Strategic Planning Insights: Identification of community needs informs the implementation and coordination of population health management solutions leveraging unique analyses for specific business needs or goals.
- Ongoing Training: Instruction on CHM Hub®’s new features and existing functionality.

How do I access the CHM Hub®?

- To access the CHM Hub®, leverage the steps below:
- Navigate to the sign-up page using a compatible browser (Google Chrome, Microsoft Edge, or Mozilla Firefox).
- Enter your information and credentials:
- Full Name = First Name and Last Name
- Company Name = Plan or Organization
- Title = Formal title within your organization
- Email = Employee E-mail address
- E-mail domain must be on list of approved domains for your Plan or Organization
- Password = Passwords must be at least eight (8) characters in length having a capital letter, lower case letter, a numeric value, and a special symbol.
- Review and accept the ‘Terms & Conditions’ and the 'Privacy Policy'
- Select 'Request Access'
- The administrator for your Plan or Organization will verify your account and approve or deny the account request. You will receive an email from chmhub@bcbsa.com indicating the outcome of your account access.
How do I reset my password?

- Click the 'Forgot Password?' link on the login page and enter the email tied to your account. You will then receive an email with a password reset link where you can set a new password.
